Would you like my book with all 382 D&D Character Survey questions?
It's no charge, print-friendly, device friendly, and bookmarked for you:
Let's start at the top with some basic survey questions. If your players build flat player characters that focus more on numbers than plot, then consider asking them some of these questions during a Session 0 or as your campaign wends onwards to kick things off.
- Give a two or three word description of yourself. (Describe you's concept.)
- Do you have any nicknames, street names, titles, nom de plume?
- What is your full birth name?
- Where do you live?
- Why do you live there?
- What is your citizenship status?
- What is your most obvious blessing or strength?
- What do you perceive as your greatest strength?
- What is your most obvious flaw or weakness?
- What do you perceive as your greatest weakness?
- Was there any event or cause of these weaknesses?
Some players might find it helpful to get a clear vision of what their character looks like and their physical traits beyond a mere strength score.
Appearance becomes important during roleplay encounters for things like NPC reactions, so this part of the character questionnaire helps you too.
- How old are you?
- What is your gender?
- What is your species/race?
- How tall are you?
- How much do you weigh?
- What is your general body type, frame, bone structure, and poise?
- What is your skin colour?
- What is your hair colour?
- What is your hair style?
- Do you have any facial hair?

Now our D&D character survey starts hitting high notes for your adventure building and campaign planning!
Player answers to these questions ground the characters in your world (no pun intended, haha) and give you nuggets of world building inspiration.
One cool approach you can take is to ask one of these questions every session and have each player give you an answer. Over the course of just a few sessions, you'll be amazed at the increased depth of your game.

No character should be an orphan. That not only gives you fewer plot hooks, but it severs a character from the world. Everyone needs a place they call home.
A mysterious past, fleeing home, or being exiled are great hooks if a player doesn't want to detail their family out. However, use this character survey section to prod for detail - or make them up yourself - so the character has roots and a stronger place in your setting.

- Would anything change your mind on this issue and if so, what?
The stronger the ties your player characters have to your world and NPCs within it, the more hooks and loops you can create in your campaign.
Leverage relationships to deliver plot hooks, provide the party resources, and create simple information networks that help keep your adventures on track.

Whew! That's a lot of questions. And a big opportunity to make your players' characters unique, deeper, and more interesting to play.
For brevity, I cut out most follow-up explanation type RPG character questions, such as “Why”, “how come”, and so on. Feel free to attach a “why” question after any item in the questionnaire to generate more details from your players.
The character questionnaire can be changed without much effort for different genres and types of details, as well. So feel free to exclude or modify survey questions and create a character survey that suits your group.
As game master, use your character questionnaire’s answers to design around character drives, roleplay off character details, and engage players beyond mere numbers.
Even just a few survey answers can inspire new adventure ideas and encounters.
To get started, pick your 5 favorite questions and email them to your players to think about. And then ask them again next session and record their answers.
Each session, pull out another question and build up character backgrounds, personalities, and world details over time.
Hook your plots to these details to reward players for answering your survey, and to create wonderful connections with the interesting people, places, and things in your campaign.
